This Command Center Staff Registered Nurse (RN) role is for an RN who wants to be part of an innovative team working as a virtual RN in a command center. The Command Center Registered Nurse will be the nucleus of the care provided to inpatients who are cared for in their home by medical personnel. This RN will have virtual visits with the patient, document in the electronic medical record, coordinate the plan of care, and participate in multidisciplinary rounds. This will all be done from a central command center where you will be working with other RNs and Medical doctors to care for patients who require inpatient care, but can receive the care in their home. Plenty of free parking! Sign-on Bonus of $5,000.00 available for experienced RN's (1+ years of experience required). Important Details: Bonuses are paid out in 3 payments: $1,666 at 30 days of hire, $1,667 at 6 months, and 1,667 at the 1-year anniversary. To be eligible, you must be a non-BILH employee or a previous eligible employee who returns to BILH after 1 year. Employee must be in good standing to receive the bonus at the time of payment. All bonuses are subject to applicable taxes. If you leave Lahey Health & Medical Center before your first anniversary you will pay back the sign-on bonus to Lahey Health & Medical Center. This program is subject to change at any point. Job Description: Reporting Directly to the Virtual Hospital Director of Nursing, The Control Center Staff RN is responsible for providing high-quality patient care to various patient populations within the framework of the Medically Home model. Essential Duties & Responsibilities including but not limited to: Utilizing monitoring, assessment, planning, intervention and critical thinking skills to care for acute care patient populations in the home Navigating patients and families across the continuum of the Medically Home phases of care Monitoring and responding to biometric data per Medically Home protocols Conducting home and audio/visual patient care visits as directed by the care team and/or protocol Ensuring all interventions in the patient’s care plan are implemented efficiently with a patient centered approach Leveraging the resources and expertise of the Medically Home team and supplier partners to implement collaborative workflows, standards, policies, protocols, guidelines and documentation systems to support safe, reliable, high-quality, evidence-based care with clinical protocols as the foundation Adhering to the clinical protocols and be willing to obtain the necessary training to provide care within the context of the providing evidence-based care Promoting professional practice and a culture of safety; willingness to engage in process improvement efforts Clinical Roles and Responsibilities: Providing protocol driven assessment, care coordination and interventions as appropriate informed by Medically Home protocols for diverse patient populations with a focus on oncology, medicine and post- operative care Exhibiting sound clinical decision making and critical thinking skills Maintain and practice in accordance with nursing guidelines per state and hospital regulations Facilitating admissions to Medically Home utilizing MH guidelines Serving as a role model for professional nursing practice Utilizing nursing knowledge to identify, prevent and/or solve complex acute and/or recurring patient care problems Sharing expertise with the multidisciplinary team and serve as a resource to staff in area(s) of expertise Supporting research efforts to advance knowledge and promote evidence-based practice Preforming other duties and responsibilities as assigned by the Senior Director of Nursing Quality and Safety Responsibilities: Ensure compliance with reporting requirements for Sentinel Events and Serious Incidents Participate in quality and safety investigations and root cause analyses Take appropriate action to ensure patient safety as a result of any investigation; and Serve as a resource in quality improvement processes and technique Organizational Requirements: Maintains strict adherence to the Lahey Clinic Confidentiality Policy. Incorporates Lahey Clinic Guiding Principles, Mission Statement and Goals into daily activities. Complies with all Lahey Clinic Policies. Complies with behavioral expectations of the department and Lahey Clinic. Maintains courteous and effective interactions with colleagues and patients. Minimum Qualifications: Education: The minimum post-secondary education requirement to become a Registered Nurse is a Bachelor of Science degree Licensure, Certification, Registration: A RN current license Skills, Knowledge & Abilities: Clinical knowledge Able to promote patient safety and a positive patient experience Flexible and comfortable with a changing and dynamic work environment and schedule across all shifts and days Team player who also works well independently Participates in ongoing learning and continuing education to care for a variety of patient populations Collaborate, advocate and mentor others with a professional, non-condescending manner Model collaborative, respectful team relationships with colleagues and customers; lead by example Strong interpersonal, inter-professional communication and collaboration capabilities Ability to analyze, interpret and problem solve quickly with well-articulated action planning Strong verbal/written communication and conflict resolution skills Strong empathy to deliver compassionate, patient centered care Experience: One and a half years of RN experience Pay Range: $37.00 - $95.74 The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
