F.A.S.T. Overnight Nurse RN/RPN

POSITION PROFILE
Access Healthcare Services is looking for an experienced full time Community RN or RPN to fill the role of F.A.S.T. (Fast Access Service Team) Nurse in Renfrew County overnights from Sunday to Thursday. This is a hybrid position comprised of work from home and travel to client homes as clinically required.
The F.A.S.T Overnight Nurse provides timely clinical triage, assessment, and nursing care to community-based clients outside of regular business hours or during periods of increased service demand. This role supports continuity of care by responding to clinical concerns via phone or virtual platforms and attending in-person visits when clinically indicated. The F.A.S.T. Overnight Nurse works collaboratively with the interdisciplinary team to ensure safe, responsive, and high-quality client-centered care.

ROLES & RESPONSIBILITIES

  • Provides on-call nursing support through clinical triage, assessment, and decision-making in response to client or caregiver concerns.
  • Completes in-home client assessments as required during on-call hours and develops, implements, and maintains collaborative, client-centered care plans accordingly.
  • Delivers nursing care virtually (telephone or video) and/or in person based on assessed clinical need.
  • Provides complete, competent, and relevant nursing care to clients in accordance with scope of practice.
  • Maintains and updates client care plans as client needs, conditions, or goals evolve, including required strategies or interventions.
  • Performs all nursing skills and related tasks in accordance with Access Healthcare Services policies and procedures and community protocols.
  • Adheres to best practice guidelines and Professional Practice Standards as outlined by the College of Nurses of Ontario (CNO).
  • Demonstrates the mission, vision, and values of Access Healthcare Services Inc. while providing care to clients and/or their families.
  • Observes, assesses, and documents client condition, changes, and responses to interventions, following documentation standards set forth by the CNO and AHS policies and procedures.
  • Practices infection prevention and control measures at all times.
  • Provides client and caregiver education, health promotion, and guidance related to care needs, symptom management, and home health goals.
  • Undertakes medication management and wound management as per best practice guidelines and AHS policies and procedures.
  • Provides palliative and end-of-life (EOL) care and support as required to ensure patient comfort and to support families.
  • Makes recommendations for devices, tools, or strategies that may improve client outcomes or quality of life.
  • Promotes self-care, independence, and autonomy for nursing and/or personal support clients.
  • Communicates and collaborates with members of the interdisciplinary health care team, including nursing colleagues, personal support workers, managers, Ontario Health atHome (OHaH) and other members of the interdisciplinary team as appropriate.
  • Reports, consults, and updates relevant members of the care team on client status, changes in condition, and treatment plans in a timely and professional manner.
  • Completes Automated Provider Reports and Automated Provider Orders as required and in a timely manner.
  • Identifies, reports, and escalates any potential or actual safety risks to the Program Manager or delegate immediately and as per process.
  • Seeks support and clinical guidance as appropriate to ensure ongoing competency and adherence to best practice standards.
  • Participates in ongoing internal and/or external education and professional development activities.
  • Provides inter-program or cross-program support during on-call coverage as directed by the Program Manager or delegate.
  • Completes other duties as assigned to support safe, effective, and timely delivery of on-call community nursing services.
QUALIFICATIONS
  • Registered Nurse or Registered Practical Nurse in good standing with College of Nurses of Ontario (CNO)
  • Minimum 3 years clinical nursing experience in home and community care is essential
  • Experience in Palliative/end-of-life care and symptom management and asset
  • Demonstrated experience in clinical assessment, triage, and decision-making, including the ability to respond effectively to urgent or changing client needs.
  • Ability to work independently and manage competing priorities during on-call hours.
WHAT YOU WILL NEED
  • Registered Nurse or Registered Practical Nurse in good standing with CNO
  • Proof of Covid-19 vaccination
  • Vehicle required with valid driver’s license
  • Car insurance with a liability of $2 million
  • Valid CPR
  • Step 2 TB Test (Completed within the last 2 years)
  • Vulnerable Sector Police Check (less than 3 months old)
