This opportunity is fully remote in Alabama, Arkansas, Florida, Georgia, Mississippi, Tennessee, Texas, Kentucky, Oklahoma, or Virginia. The Chronic Care Management role is full-time, Monday to Friday during the day. As part of the team, you'll be giving personalized attention to patients with chronic conditions over the phone. Your tasks will include gathering health info, offering guidance, and promoting treatment adherence. LPNs have the potential to make up to $23 per hour and CMAs can earn up to $17 per hour. We'll hook you up with all the necessary tools, you just need internet access and a quiet arenaflex at home. Requirements: • Must be an LPN or CMA. • Need to have high-speed internet (no satellite or hotspots). • Proficient in arenaflex Outlook and Excel. • Experience with EMR documentation. • Self-driven, comfortable with remote work, and enjoy assisting others. Responsibilities: • Perform initial risk assessments to develop personalized Care Plans for patients. • Work closely with patients to strategize and oversee their care. • Encourage adherence to appointments and medications. • Serve as a patient ally and provide education on chronic illnesses.
